Spot On Massage LLC Policies

Thank you for choosing Spot On Massage LLC as your massage and bodywork provider. In order to provide the best service possible to our clients, we have implemented the following policies:

Cancellation & No-Show Policy:

We require at least 48 hours' notice to cancel an appointment. We need those hours to book another client in that time slot. Clients who cancel an appointment with less than 48 hours' notice will be billed 100% of the price of the scheduled service to protect the therapist’s time and schedule. Clients who do not show up for a scheduled appointment or notify us in advance will be billed 100% of the scheduled service. The payment is due within 7 days of the missed appointment. We understand that emergencies and illnesses can arise, therefore last-minute cancellations due to things such as verifiable emergencies, illnesses or inclement weather will generally not result in any missed session charges. However, this is to be determined at the therapist’s discretion.

Late Arrival Policy:

All scheduled appointments will end at the scheduled ending time in order for us to stay on schedule. Clients who arrive late to their scheduled appointment will be charged for the full session and will not receive a time extension. For your first appointment, please arrive 15 minutes prior to your scheduled appointment time to allow time to complete the Client Intake Form. For all other appointments, please arrive 5 minutes before your scheduled appointment time to allow time to undress and get on the table to enjoy a relaxed and unhurried experience.

Massage Termination:

Only professional massage and bodywork services for relaxation or therapeutic purposes are offered at this massage practice. Massage services will be terminated immediately in the event of inappropriate conduct of any kind. This includes harassment, threatening speech or behavior, sexual advances or requests, or disrespectful actions or language. A session will not be conducted if the client is under the influence of drugs or alcohol. If the massage is terminated for any of these reasons, full payment for the scheduled session is still required. 

Client Termination:

Clients may also terminate a massage session at any time.

Draping Policy:

Clients will be appropriately draped with a sheet and/or towel at all times during their massage. Only areas of the body that are currently being treated will be exposed. The breast and genital areas will always remain draped and are never massaged. 

Consent Form For Professional Massage Therapy And Liability Waiver: 

It is my choice to receive massage therapy. I am aware of the benefits and risks of massage and give my consent for massage. I understand that there is no implied or stated guarantee of success of effectiveness of individual techniques or series of appointments. I acknowledge that massage therapy is not a substitute for medical care, medical examination, or diagnosis. I have stated all medical conditions that I am aware of and will inform my practitioner of any changes in my health status. 

CORONAVIRUS/COVID-19 Risk And Waiver Of Liability: 

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Spot On Massage LLC, and its employees, contractors or therapists cannot guarantee that you or your family will not become infected with COVID-19. Further, by receiving a massage, you could increase your risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by receiving a massage and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Spot On Massage LLC may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, and their families. I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any injury to myself, my family, and my contacts (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I, or they, may experience or incur in connection with my attendance at Spot On Massage LLC. On my behalf, and on behalf of my family, friends, and contacts, I hereby release, covenant not to sue, discharge, and hold harmless and indemnify Spot On Massage LLC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Spot On Massage LLC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Spot On Massage LLC appointment.
